MedicareCard-JohnSmith[1]Currently the only Insurance program our office participates with is Medicare. We do not accept any other insurance, or Medicare Advantage.

If you are on Medicare and have additional insurance, Medicare MUST be primary. Please note that we do not participate with any 3rd party insurance programs, including Medigap plans. Therefore it is expected that any patient with Medicare will be responsible for copayments and deductibles AT THE TIME OF SERVICE. Once your Medigap insurance provides payment for services, we will credit your account with any monies paid. If you are unwilling to provide payment at the time of service, you have the option to fill out a credit authorization form which we will hold up to thirty (30) days from time of Medicare payment pending payment from your secondary.

If you have private insurance, no insurance, or participate in a Medicare Advantage plan, we are still happy to see you. Payment is required in full at the time of service. We will be happy to provide you with a copy of the service bill so that you may submit to your insurance for out-of-network reimbursement.

We’re sorry but we are unable to see patients with Medicaid (as either primary or secondary insurance) at this time.

We are happy to accept Cash, Checks, VISA, MasterCard, Discover, and American Express. We are unable to accept HSA/FSA cards. Please note that a processing fee does apply to credit transactions (3% for in person swiped transactions, 4% for all others).
